Is this the official LPA form?
No. You still make the LPA on the official OPG form (online or LP1H). This pack helps you make the decisions and complete it correctly.
When can my attorney use it?
Only once it is registered and only when you lack mental capacity to make the particular decision yourself.
Should I also make a financial LPA?
Many people make both. They cover different decisions and each needs its own form and fee.
Is an advance decision the same thing?
No. An advance decision lets you refuse specific treatment in future; the pack explains how it fits with an LPA.
